The Form 4 included an option to purchase 10,000 shares of common stock of the Issuer (the "Common Stock"); however, the grant was for an option to purchase 15,000 shares of Common Stock. Accordingly, the previously filed Form 4 omitted the option to purchase an additional 5,000 shares of Common Stock. The option to purchase an aggregate of 15,000 shares of Common Stock shall vest in 1/3 increments over a three year period commencing on the date the Issuer receives shareholder approval of the Issuer's 2016 Equity Incentive Plan.
